This is a dental savings plan, not dental insurance. It cannot be used with another dental plan; for services covered by workers’ compensation, auto, or medical insurance; for referrals to specialists; for hospital charges; or for implant, implant-related, or sedation services. Treatment outside the scope of the treating dentist is excluded. Membership fees are non-refundable. Patient portion is due at time of service.
